AUDIOVISUAL RELEASE FORM

            I hereby give to DECA (Department of Education and Cultural Affairs) the right to copyright, use, produce, publish, publicly distribute and reproduce audiovisual works in which I may be included in whole or in part, including the use of printed matter in conjunction with the audiovisual works. 

            I make this donation in return for the premise of DECA to use the audiovisual works for educational purposes and not for commercial exploitation. 

            I waive my rights to inspect and approve the finished audiovisual works or their eventual use.

            I warrant that I am fully competent to contract in my own name, and that I am signing this of my own free will.  I have read this release and I fully understand that I am giving up any rights I may have to the audiovisual works covered by this release. 

            I understand that this agreement is binding to me, my heirs, executors, administrators, personal representatives and assigns.

                                    CONSENT FOR A MINOR

            If the person giving the release is under 18 years of age, the parent or guardian should also consent to the release:
